Job Summary
Main purpose of the job:
- Support patients to navigate the health care system
Location:
- Lillian Ngoyi Clinic and Stretford Clinic – Soweto, Johannesburg
Key performance areas:
- Provide cancer health education to patients/clients
- Conduct cancer screening for clients (Use Screening Tools with questions- mainly at PHC)
- Establish good relationships with patients and families, patient care teams, and clinical staff
- Communicate with health care teams to facilitate patient care
- Identify patient barriers to care and refer to appropriate resources
- Maintain patient confidentiality
- Navigate patients within the health care system (Know your referral system)
- Follow up with patients to ensure there is no loss to follow up, including the following:
- Identify patient’s priority needs and refer appropriately
- Meet with the palliative care team regularly to discuss patient issues where indicated
- Maintain records of patient interactions
- Report all challenges that you are experiencing to be offered support
Required minimum education and training:
Required minimum work experience:
- Minimum one year of working experience
Desirable additional education, work experience, and personal abilities:
- Previous community work or volunteering as a community worker will be an advantage
- The ability to capture data on Redcap would be an advantage
- Good cultural understanding of the population served by the hospital
- Basic computer literacy
- Good communication skills
- Basic understanding of the health care system
- Trauma and counseling skills an advantage
- Ability to build relationships with different patients and service providers
- Commitment to good patient care
- Empathetic, passionate, and trustworthy when working with patients
